Neurological Patient ER Visit Checklist

A free two-page printable for caregivers heading to the ER with someone who has a brain injury or neurological condition. Page one covers the patient's details: insurance, medications, diagnoses, allergies, medical team, safety needs and implanted devices, a VA and veteran care box, and a "what's normal for this person" baseline section so ER staff can see what has actually changed. Page two is a guide for ER staff covering TBI, CTE, Parkinson's and parkinsonism (including PSP and MSA), FTD, dementia, Lewy body, FND, PTSD, and aphasia. It also has a medication safety warning about drugs that can be dangerous for Lewy body and Parkinson's patients. It prints clearly in black and white, so print as many copies as you need.
